What the new Mental Health Service User Feedback Questionnaires are

The new feedback questionnaires give people using Mental Health Services the opportunity to share their experiences of specific services in a simple and meaningful way.

Each questionnaire is tailored to a particular Mental Health service area, helping us understand what is working well and where we can improve.

Why we are asking for feedback

Listening to people who use our services helps us to improve care.

Your feedback helps us to:

  • Understand what matters most to you
  • Identify areas where services can improve
  • Build on what is already working well

We are committed to using feedback to improve Mental Health Services and to sharing how it leads to change. 

How the new questionnaires differ from the Friends and Family Test (FFT)

They are different but complementary.

  • The Friends and Family Test (FFT) asks a short, organisation-wide question about overall experience

  • The new questionnaires ask more detailed questions about specific Mental Health services

Together, these help us build a clearer picture of people's experiences and improve services where it matters most.

Completing the questionnaires is voluntary

Taking part is completely voluntary.

If you choose to complete a questionnaire, your feedback will help us improve Mental Health Services for others in the future.

Confidentiality

Your feedback is confidential. 

  • Your feedback is collected securely
  • It is used for service improvement
  • Information is reported in a way that protects individual privacy

We will never identify people when sharing feedback or learning.

How long the questionnaires take to complete

The questionnaires are designed to be quick and easy to complete, while still giving you enough space to share what matters to you.

You can skip any questions you do not wish to answer.

How your feedback will be used

Feedback will be:

  • Reviewed by Mental Health services
  • Used to improve care and service delivery
  • Combined with other feedback to identify themes and learning

We are committed to showing how feedback leads to real improvements through future updates.

Who can complete the questionnaires

Anyone who has used Mental Health Services, or who is supporting someone as a carer, may be invited to share feedback using the relevant questionnaire.

When the new questionaires are available

The questionnaires are publicly available from Monday 8 June 2026.

You can access the new questionnaires through the Mental Health Service webpage, where you will find:

  • A link to each service-area questionnaire
  • Information about how feedback is used
  • Support and contact details if you need help

If you have questions or need support, you can:

  • Speak to a member of staff
  • Contact Mental Health Services through the usual channels
