What are long-term neurology conditions
Long-term neurological conditions are disorders that affect the nervous system (brain, spinal cord, nerves) and continue over a long period of time. These conditions can affect movement, sensation and other bodily functions.
About our service
Services to people with long term neurological conditions are provided by occupational therapists and physiotherapists. Our aim is to enable individuals to carry out activities of daily living that are important to them, as safely and as independently as possible.
We work together to treat and support patients with neurological long term conditions and their carers. We work to achieve agreed goals, with the aim of maximising potential and participation in the local community. We will help you to identify difficulties in your everyday life and try to find practical ways in which you can become more independent in these activities. This can be through the process of rehabilitation, provision of treatment strategies, or by advising on appropriate care.
Occupational Therapy
We offer a range of services to individuals with long term neurological conditions. If you are struggling with everyday tasks due to a physical problem, fatigue or problems with your thinking skills, we provide services to assist.
Physiotherapy
This service provides assessment for movement disorders associated with neurological dysfunction. We work closely with the inpatient services for stroke, including the Spasticity Management Clinic which is run by a consultant and specialist physiotherapist. We aim to provide patients from this clinic with a follow-up session for a physiotherapy review and advice.
We also work with the specialist nurses for Stroke, Parkinson’s Disease and Multiple Sclerosis and the long-term conditions co-ordinators. These services encompass condition specific education as well as liaising with other services to manage the care of complex conditions.
How to access this service
Patients can be referred to our team by any relevant health or social care professionals. We also accept self-referrals for patients who have been known to the service previously. Referrals are then triaged by an appropriate clinician and appointments allocated according to the patient's needs.
What will happen once I have been referred
Your referral will be placed on a waiting list. Do not be concerned if you do not hear anything immediately. You will be sent an appointment or contacted by telephone to arrange the time and date of your assessment.
Where can we see people
Typically, we see patients in clinic rooms at Central Community Health Centre, in their own homes, in the local community or in the therapy gym at the Health Centre. Patient transport may also bring people into the Health Centre for treatment. It very much depends on the patient needs and the most appropriate environment to meet clinical needs.
What will happen during my first physiotherapy appointment
Your first initial appointment will last approximately 60 minutes. We will record any medications you are currently taking, so please bring a complete list of your medications with you.
You should wear something comfortable so you are able to move freely.
A treatment plan will then be discussed and agreed with you.
This can include:
- Advice
- Home exercise programme
- A block of exercise sessions, a physiotherapist or a specialised therapy assistant will carry these out with you
- Onward referrals, for example, to an exercise class in the community
What will happen during my first Occupational Therapy appointment
Your initial appointment could last up to 90 minutes. This can be in your own home or as an outpatient. We will record any medications you are currently taking, so please have a complete list of your medications available.
A treatment plan will then be discussed and agreed with you.
Occupational Therapy sessions will vary dependent on your needs and priorities but may include:
- Fatigue management
- Rehabilitation, physical or cognitive
- Provision of aids and adaptations
What happens if I cannot attend the appointment

You need to be available to attend a block of therapy in order for the treatment to be beneficial.
What will happen after my treatment
After your course of treatment, a discharge letter will sent to the healthcare professional who referred you. The letter will detail your therapy sessions and the outcomes of the treatment.
It is important that you continue with the exercises your therapist has given you as part of your everyday routine to manage your condition.
There are various schemes on island where you can continue to exercise which your therapist can refer you on to. This can be further discussed with your therapist upon discharge.
