How to use these forms

Forms are provided in Microsoft Word, Adobe Acrobat (PDF) format or both.

If you would like to complete the form online and save to your computer use the Microsoft Word version of the form. If you would like to print the form and complete in handwriting use the PDF version.

To use the Microsoft Word version:

  1. To save the form to your hard disk, click on the required form. Some browsers allow you to view Microsoft Word documents within the browser. If your browser opens the form and you want to save it to your hard disk, right-click on the file icon and select 'Save Target As...'
  2. Only those sections of the form to be filled in are accessible. Using the TAB button the cursor will move from one fillable section to the next. If you click the cursor in a section and select the information text in it you can then type or paste your data over the selected text. There is no limit to the amount of data you may place in any section.

To use the Adobe Acrobat (PDF) forms:

For best results you need to ensure you have Adobe Acrobat Reader 5 or higher. To receive the latest version of Adobe Acrobat select the link below.

  1. To save the PDF form to your hard disk, right-click on the file icon and 'Save Target As...’ To open the form within the browser, click on the file icon.
  2. You may complete the PDF form in two ways;
    1. By printing it and filling it in manually, or
    2. By filling it in “on screen” and then printing it. Please note that you cannot save what you have filled in on the PDF.
To fill in the form 'on screen':-
Completed forms should be submitted to the Courts in the normal way.

Frequently Asked Questions

QMicrosoft Word forms are locked and I can't change the main text of the form.
AThe Microsoft Word forms are deliberately locked so that users of these forms are able to alter only those parts of the forms that are required to be completed.
QMicrosoft Word forms do not print out to full A4 size.
AInitially forms are not full page length if printed out. However, once the fillable portions are completed the pages do fill out. Microsoft Word versions are intended to be used by people directly typing them and not for printing out and filling in manually.
QThe text size on the Microsoft Word form is too small.
AIf you have concerns about the font size, open the document at 120% plus.
QWhen I try to download/print the PDF form I encounter problems.
