This electronic form can be completed by all applicants who have access to a supported device (please note that they have to support Abode Flash and as such is not supported by Apple devices).
This form allows you to fill in the application on your computer and will help to make sure you complete in the correct sections.
Once you have completed the form you need to be able to print it.
If you require a countersignature, they must fill it in after you have printed the form.
This is a form that has been created for those applicants who wish to complete their form on a computer. It will ensure that you fill in the correct sections of the application form.
How to fill out the form
The electronic form must be completed on an Adobe flash supported device, such as a desktop, laptop or android-based mobile. It is not supported on iPhones or iPads.
Printing the form
The electronic form will not print if the form is incomplete. Please check the application.
If you do not have access to a printer, you will not be able to complete the electronic form. The Welcome Centre at the Sea Terminal has a ‘hub’ that will allow you to complete the application form and print once you have finished.
Paper forms are still available from Post Offices, the Welcome Centre and the Passport Office.
Submit the form
The form must be completed, printed, signed (and complete by the countersignature if applicable) and then sent to our office via post or in-person. Please ensure that you save the form.
The form cannot be submitted by email.

- Reduce processing times as when the big barcode on the front of the form is scanned, it will auto-populate the passport system
