Acute Medical - Ward 6, 8 and 9
This team cover the medical wards including Wards 6, 8 and 9. Patients under a medical consultant but 'outlying' on another ward may be seen by the team if occupational therapy intervention is indicated. The main aim of the team is to promote independence in activities of daily living and to support safe discharge home from hospital.
This may include:
- assessment of transfers (getting on/off bed, chair and toilet), assessment of personal care and/or kitchen activities. Home assessment can be completed where there are complex needs
- assessment of need for equipment and provide what is needed for safety at home
- follow up post discharge for up to four weeks if required. After this time please contact the Community Adult Therapy Team
- liaison with Social Care Services
