Neurological Long Term Conditions

Services to people with long term neurological conditions are provided by occupational therapists and physiotherapists. Our aim is to enable individuals to carry out activities of daily living that are important to them, as safely and as independently as possible.

We will help you to identify difficulties in your everyday life and try to find practical ways in which you can become more independent in these activities. This can be through the process of rehabilitation, provision of treatment strategies, or by advising on appropriate care.

We also support carers who are looking after adults with a physical disability or illness.

Occupational Therapy

We offer a range of services to individuals with long term neurological conditions. If you are struggling with everyday tasks due to a physical problem , fatigue or problems with your thinking skills, we provide services to assist

Physiotherapy

The Long- term Conditions physiotherapy service is a small team of specialist neurological physiotherapists who assess and treat patients with neurological conditions. We offer an outpatient service based in the Community Health Centre and may carry out home visits as appropriate. Patient transport may also bring people into the Health Centre for treatment.

We work closely with the inpatient services for stroke, including the Botulinum toxin injection Clinic which is run by a consultant and specialist physiotherapist.  This service provides assessment for movement disorders associated with neurological dysfunction. We aim to provide all patients who receive botulinum toxin injections with a follow-up session for a physiotherapy review and advice.

We also work with the specialist nurses for Parkinson’s Disease and Multiple Sclerosis and the long-term conditions co-ordinators. These services encompass condition specific education as well as liaising with other services to manage the care of complex conditions.

If you are unable to attend an outpatient appointment your physiotherapy will usually be provided by the community team who visit people at home.

How to access 

For referral to the outpatient neurological physiotherapy service you will need to be referred by any of the following

  • GP
  • Physiotherapist
  • Specialist Nurse
  • Consultant
  • any other Healthcare Professional

For occupational therapy and physiotherapy services in your own home

If you can't come to us because you have mobility problems, we will visit you where you live. Home Visits are undertaken by the Community Adult Therapy Service (CATS).
