Live Well - Stay Well
The Live well - Stay Well course is for anyone over the age of 18 with a long term health condition. These can include diabetes, heart failure, ulcerative colitis, incontinence, stroke, asthma, epilepsy, arthritis, chronic pain, crohns disease, multiple sclerosis, Parkinson's, kidney disease and high blood pressure - and many more.
The Live Well - Stay Well course is a free 6 week programme that aims to encourage people with long term conditions to manage their condition by developing and applying new skills. With an emphasis on self care, the course covers a new skill each week, ranging from improving communication with those around you to ways of coping with pain and fatigue, the importance of maintaining a well balanced diet, and identifying the need to plan for the future. The course helps people to gain more confidence in managing their condition, introduces people who face similar problems, and creates an environment in which people are able to share difficulties and find new way of dealing with them.
By teaching a variety of new skills and approaches, the course has left 100% of participants feeling more positive about living with a long term condition, and over 85% feeling more confident about their everyday activities, talking to health care professionals and their ability to participate in social events. All participants are also given, free of charge, a copy of 'Self-Management of Long-Term Health Conditions - A Handbook for People with Chronic Disease' to take home.
From its beginnings in 2009, the Live Well - Stay Well course has been helping those with long term living conditions to manage their conditions on a daily basis through the development of new skills.
