Stroke Review Clinic

This is led by the lead nurse for stroke and support by occupational therapy. It is for all patients diagnosed with stroke and aims to provide follow for a period of 3 -12 months post stroke, depending on need. It offers expert assessment of:

Functional ability
Medication (in relation to stroke)
Mood
Continence
Lifestyle
Neurological and clinical assessment

You will be referred by the stroke unit on discharge but can also be referred by any other healthcare provider. Initial assessment aims to be 6 weeks post discharge.
