Heart Failure Service

The Heart Failure service is designed for those people who have impaired heart function, which results in symptoms of breathlessness and fluid retention.

The team is based in the Medical Outpatients Department, situated on the Noble’s Hospital site at Braddan.

Opening times

Outpatients department Monday to Friday from 8.30am until 5pm (excluding bank holidays).

The heart failure clinics run on an appointment only system, but access to the heart failure nursing service is available via the telephone between 9am and 5pm. Messages left after 5pm will be returned on the next working day. Please remember to leave your name and number clearly when you call.

Locations where the service is provided

The service can be provided in the patient’s own home, in care homes, hospital clinics and other community settings and during hospital admissions.

The Team

The Isle of Man Heart Failure Service is made up of the following staff:

  • Consultant Cardiologist
  • Staff Grade Doctor
  • Cardiac Specialist Nurse
  • Administration support

The service is led by the Cardiology Consultant, working with the Staff Grade Doctor, the Nurse Specialist and Secretarial teams. For patient convenience, a “one-stop approach” to clinic attendances has been adopted. This means that on a single visit, the patient may see a number of health care professionals, thereby reducing the number of visits required.

Service Aims

  • To identify and reverse any cause of heart failure, where possible
  • To provide evidence-based treatment
  • To improve quality of life by improving symptoms or by slowing their deterioration
  • To reduce the length of hospital stays
  • To reduce re-admissions to hospital
  • To reduce mortality rates
  • To improve the End of Life experience for patients and their carers
  • To provide high quality, timely, individualised care

Outpatient Service

The heart failure team runs appointment only clinics. We do not offer a drop-in service. All people attending the Outpatients Department should book in with the Welcomer on arrival.

People attending for appointments are asked to bring with them:

  1. A copy of their prescription, listing all of their current medication
  2. Their personal record diary

Consultant-led clinics and Staff Grade Doctor-led clinics take place 3 times per week at both Noble’s Hospital and Ramsey Cottage Hospital.

Specialist nurse-led clinics take place once a week at Noble’s Hospital and once every other week at Ramsey Cottage Hospital.

Appropriate patients are referred to Specialist Hospitals in the UK (Tertiary Care), for further investigations and treatments.

In-hospital Service

Our aim is to prevent hospital admissions with heart failure. Should you be admitted to hospital with deterioration in your condition, we will endeavour to review you on the ward and provide you with advice regarding your treatment.

Once discharged from hospital, follow-up appointments will be provided to ensure your condition is re-assessed to aid in ensuring symptoms remain stable.

Heart Failure Specialist Nurse Service

There is 1 dedicated heart failure specialist nurse who delivers cardiac care, education and support across the Isle of Man.

Services delivered by the heart failure specialist nurse include:

  • Education for people newly diagnosed with heart failure and for their families and carers
  • Ongoing education and support for people with heart failure and for their families and carers
  • Support for healthcare professionals across the Isle of Man in heart failure management, in a variety of care settings
  • Individual therapies initiation in order to improve control of symptoms
  • Education for student nurses / overseas nurses in conjunction with the School of Nursing
  • Education for healthcare professionals across the Isle of Man

People with heart failure managed by their GP are asked to contact their GP or Practice Nurse for advice and support. If necessary, the GP should refer to Dr Tibitanzl via the Consultant Referral System.
