The services focuses on vulnerable adults who have complex health needs which do not require input from the District Nursing Service in order that they may stay safe, be healthy, enjoy and achieve, make a positive contribution and prosper.
A vulnerable adult is a person aged over 18 years or over ‘who is or may be in need of community care services by reason of mental or other disability, age or illness and who is or who may be unable to take care of themselves, or unable to protect themselves against significant harm or exploitation’.
The service operates Mon – Fri 09:00hrs – 17:00hrs, except Bank Holidays.
Continence Advisory Service – Tel: +44 (0) 1624 693551
This service exists to provide professional advice, support and information to all individuals with continence problems, or their carers. Where continence cannot be retained or regained, advice on, and provision of suitable aids and appliances is included. All local residents have the right to access the service, either through regular clinic sessions held throughout the Island or via home visits.

The Parkinson’s Disease Nurse Service provides specialist care to people with Parkinson’s Disease. Patient are seen in both Primary and Secondary care according to their individual requirements. The service provides information, education and support to both the clients and carers at any stage of their illness, and works to ensure that they receive not only optimum care but also improved quality of life. The Co-ordination and initiation of early referrals to the multi-disciplinary team generates the link with other professionals who can make a positive impact on patients’ lives and the lives of their carers.

Transfer of Care Co-ordinator – Tel: +44 (0) 1624 650722
The Transfer of Care Co-ordinator coordinates the care of clients with complex health needs, including patients who have Acquired Brain Injury. Packages of care can be arranged for those returning to the Island following treatment in the UK.

Prison Healthcare Service – Tel +44 (0) 1624 891076
A healthcare department is situated in the prison and provides healthcare to prisoners from 07:30 – 20:30 hours Monday – Friday and 08:30hrs – 17:30hrs hours at weekends. The service is staffed by a multidisciplinary team. In addition to general healthcare the team offers advice on a range of health issues and actively engages in health promotion activities on a range of topics.
