Community Support Service
The Aim of the Community Support Service is to:
- Provide varying levels of support for Service Users who are unable to live at home independently, but who do not require residential or nursing care.
- Work with Service Users to maintain their ongoing independence.
- Support Service Users in a way which enables them to do as much as they can.
- Improve and maintain quality of life for people receiving support.
- Provide a bespoke package of support to anyone eligible for the Service.
- Delay the need for residential care or hospitalisation, therefore minimising the whole life cost of care.
- Ensure the Service User is at the centre of service delivery with a focus on promoting their skills, abilities, and individual strengths, enabling them to be as independent as possible within their home environment.
- Provide an environment in which positive risk taking is encouraged and the negative consequences of risk are assessed, managed and reduced.
The Service can be requested for:
- People who have completed a Reablement goal package, and need ongoing support for activities of daily living.
- People, who through accident/illness, need support to maintain their independence in the longer term.
- People whose level of ability results in a risk to their independence, health and wellbeing being maintained.
- People whom as a result of their needs, are unable to achieve one or more of the following outcomes:
- Managing and maintaining nutrition for a healthy diet,
- Managing shopping, whilst exercising choice over groceries and necessities,
- Maintaining own health (medication, appointments, personal hygiene, etc.)
- Managing hygiene and continence needs with dignity,
- Being appropriately clothed and groomed, and able to feel pride in their appearance,
- Being able to independently mobilise around their home safely.
The Service can support in a wide variety of ways, including, but not limited to:
- Medication prompting or administration
- Support with personal hygiene, washing, dressing
- Supporting mobility, transferring, use of hoists and various aids
- Aiding continence needs
- Supporting those with Dementia, Parkinson’s and various other conditions
- Food preparation including specialist dietary needs
- Emotional wellbeing
- Shopping
